Registration Now Closed For Boys 2008 Season!!

Instructions:

Please fill in (online) the shaded areas of the Player Information, Medical Information, Medical Release and Emergency Contact forms and then print them.  The fourth form is the US Lacrosse application.  Just print that and then fill in. 

Do not send money for US LAX form, it is included in the dues.

(Don't forget parent signature on US LAX form.)
